When to start
It’s recommended to perform perineal massage 3–4 times per week for 5–10 minutes starting from week 35 of pregnancy.
Before the massage
Warm your body to relax muscles and skin. A warm bath, shower, or even a herbal sitz bath can help. Trim your nails so you can massage comfortably with your fingertips.
Positioning
Lie down in a comfortable position, resting on a pillow. For your first session, have a mirror nearby to help with orientation. Place your legs wider than shoulder-width and take a few deep breaths.
How to massage:

Apply a few drops of perineal massage oil to your fingertips and gently coat the perineum and lower vaginal area.

Insert a well-oiled thumb 2–4 cm into the vagina.

With your index and middle fingers, massage the outer perineal area at the same time.

Press down toward the anus and massage the inner perineum and vaginal walls in a U-shaped motion. You should feel a mild stretch.

Hold the stretch for 1–2 minutes and repeat the U-shaped movements for a few more minutes. If you have a scar from a previous birth, this massage can help soften scar tissue.
Important tips

Perineal massage should never be painful. A slight stretching sensation at first is normal and should ease over time.

Stop the massage if you experience pain, bleeding, or suspect an infection, and consult your healthcare provider.

Benefits
Perineal massage increases blood circulation and skin elasticity, preparing your body for birth. It helps you connect with your body and can reduce the likelihood of tearing during delivery, though it does not guarantee prevention.
